  • Encourage and facilitate community engagement, enabling clients to participate in social and recreational activities.
  • Maintain accurate documentation and reports on client progress and any incidents that occur.

  • Previous experience in a similar role or relevant field is preferred but not mandatory.
  • Strong communication skills and the ability to establish rapport with clients and their families.

 JOB TITLE: Facilitator/Supervisor

Reports to: PROGRAM DIRECTOR

 

SUMMARY:

This position is responsible for providing support services and training for consumers, providing direction to Personal Care Attendants, and providing related duties as assigned.

 

ESSENTIAL DUTIES, RESPONSIBLILITIES AND RATING

 

  1. Participates with Program Director and Assistant Program Director in developing, implementing and maintaining effective consumer services in compliance with Inspiration House Care Providers, Inc. and funding source licensing requirements.
  1. Assists consumers with developing skills to fully participate in community life and accessing service to meet their needs.
  1. Functions as primary liaison to governmental agencies that provide financial assistance and services to the consumer.
  1. Completes and submits documentation of service provision within approved timelines.
  1. Completes written discharge summary for consumers who are discharged from the Program within appropriate timelines.
  1. Provides direction to assigned Personal Care Attendants by providing training, orientation, counseling and discipline according to IHCP, Inc. policies and procedures.
  1. Develops Plan of Care to meet the needs of the consumer.
  1. Provides twenty-four hour on-call coverage to consumers from Monday through Friday and participates in a weekend/holiday direct response on-call schedule.
  2. Serves as staff  backup when a PCA calls out and no other PCA is available.
  3. Performs related duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

This position requires the incumbent to have completed high school and have four years of experience working with individuals who are developmentally disabled. Post high school/college course work strongly preferred. Completion of a four-year degree in a human service discipline may be substituted for experience. Demonstrated proficiency in developing and writing care plans, ability to advocate for consumers, have EXCELLENT written and verbal communication skills, be a self-starter and have excellent organizational skills. Must be able to communicate in a clear, concise and professional manner.

 

PHYSICAL REQUIREMENTS While performing these duties the employee is required to perform the following physical tasks when the need arises: • Lift and/or move up to 25 pounds • Use hands to handle, and/or feel objects, tools, and controls • Stand, walk, sit, climb stairs, balance, stoop, kneel, crouch and crawl • Talk and hear • Specific vision abilities include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus •

 Drive a car The employee may be required to stand, stoop, bend, and assist in the transfer or lifting adults

 

SPECIAL REQUIREMENTS

Will need dependable transportation to visit consumers or assist consumers in accessing community services. • The vehicle you will drive in the scope of the Facilitator job must be registered in your state of residence • The vehicle must have a current brake tag/vehicle inspection decal. • Must have a clear Motor Vehicle Record. • Current Driver License in state of residence • Successfully complete Criminal Background Check

 

ESSENTIAL JOB FUNCTIONS:

Provide direct assistance with daily and personal adjustments; and incidental household maintenance: provide direct assistance and relief to families; Report changes in the individuals condition and needs;  Supervise individual needing assistance/care;  Assist with incidental household maintenance;  Assist with medications that ordinary self-administered, if necessary;  Accompany and assist individual in accessing community resources, doctors’ appointments, shopping and related activities; may require driving to/from;  Provide assistance with correspondence and bill paying;  Assist with household services essential to health care to home or performed in conjunction with assistance in daily living or respite care;  Assist with personal maintenance, e.g grooming, bathing, dressing, etc.;  Complete required reports, records and paperwork;  set up CPR and first aid as needed;  Serve as client advocate to ensure clients civil and human rights are protected;  Attend and participate in agency and job-related meetings and training;  Provide teaching and support to DSW’s;  Will fill-in, when a shift has not found proper coverage.

 

RESPONSIBILITY:

Work is assigned by Director and may consist of detailed instructions.  Assignments are usually carried out using standardized procedures.  When guidelines do not exist, good judgment should be exercised in selecting and adapting methods/procedures to fit usual or complex situation, and Director is to be advised/consulted on concerns about new/unique tasks. Essential job functions (expectations) are subject to change per state relations and/or client program needs.
